Chapter 6 Summary
• Explanatory conceptualizations answer the question “Why does this keep happening to me?”
• Explanatory conceptualizations generally emerge from a four-stage process:
— Gather and describe several specific examples.
— Identify specific triggers and maintenance factors by collaboratively looking for themes and commonalities among these examples. Evaluate triggers and maintenance factors in the light of existing evidence-based models for the presenting issue and “fit” to subsequent client experiences.
— Select interventions based on the explanatory conceptualization.
— Review and revise the conceptual model based on observations of the effects of chosen interventions. Does the model “fit,” and do the interventions help as expected?
• These four steps are repeated for additional presenting issues. Therapist and client consider whether the presenting issues are linked by common triggers and maintenance factors.
• The triggers and maintenance factors are considered in relation to the onset of the presenting issues to determine whether the themes present in the here and now were evident at the onset.
• Ultimately the value of an explanatory conceptualization is judged in terms of whether it leads to interventions that reduce client distress and increase client resilience.
OCD가 이미 자기 생각이나 행동의 비합리성을 알기 때문에 다른 사람은 어떤지 물어보게 하는 행동실험을 통해 머릿속지식이 아닌 몸으로 아는 지식으로 만드는 게(207-208) 인상적이었어요. (ex. 다른 사람도 비슷한 경험을 한다는 걸 알게 되면서, 자신의 경험이 보편적이지만 그에 대한 반응은 서로가 다르게 나올 수 있음을 깨달음.)
그리고 건강염려에서도 그런 일이 발생하지 않을 것이라는 설명은 도움이 안 되고 그런 일이 발생하여 사회적으로 비판받는 것이 두려운 것일 수 있다는 대안적 설명을 전하는 것이 유용할 수 있다는 부분도 좋았고(212),
OCD와 건강염려가 책임감/회피/check&ruminate 라는 공통분모를 갖기에 이를 통합된 사례개념화로 이어나가는 부분이 치료자의 역량으로 느껴졌어요(213).
치료 초반에 이런 개념화를 치료자가 끝마친 상태였다 하더라도 내담자와 함께 개념화가 실제로 맞는지 차근차근 하나씩 검증해 나감으로써 내담자 관여를 높이고 임상가의 오류를 바로 잡는 기회를 얻게 되는 것 같고, 이 책에서 누차 강조하는 협력적이고 경험적인 사례개념화가 무엇인지 다시금 배울 수 있었습니다(214).
